How can we get help from you?

Starting the Medicaid Process
STEP 1:
Have your primary care provider write a prescription and document that these services are medically necessary. Your clinic or service coordinator will provide a list of approved local agencies, of which Truth Professional Care Services is one. See more info here.

STEP 2:  Contact Truth Professional Care Services to set up a call or an in-home visit. Our representative will use a screening tool to decide if you need services and, if so, the number of hours Medicaid will cover each week. Be Realistic! Before your evaluation, think about all the things you do for you and how much time these tasks take. You know yourself best and your input is very important in this process.

STEP 3: After your eligibility has been confirmed, your Truth Professional Care Services representative will meet with your family to make a care plan. Your Truth representative will also complete prior-authorization paperwork and contact your doctor so Medicaid covers the cost of these services!

What primary services do you offer?

As a licensed and authorized LA Department of Health and Office of Aging and Adult Services (OAAS) support agency, we will work with you to implement your plan of care to help you be as independent as possible in the community. The Plan of Care is provided thru Medicaid "Waivers", which are a menu of Support Services available to you.

Which Medicaid Waivers do you provide Services for?

We provide Support Coordination and Services for the following Medicaid Waivers:
> Community Choices Waiver (CCW)
> New Opportunities Waiver (NOW)
> Long Term Personal Care Services (LTPCS)
> Residential Options Waiver (ROW)

Yes, we are state licensed for every one of the Service areas we support.
