A service for individuals between the ages of 0-21, in which periodic screenings are performed and given the health care diagnosis, the treatment can be performed.

Provides alternative services to individuals which assists them to live in the community instead of nursing facility or institution.

Provides services in the home and in the community to individuals 0 through 20 years of age, who currently
live at home with their families or who will leave an institution to return home.

To utilize the principles of self determination to supplement the family and/or community supports while supporting dignity and quality of life.

Provides ongoing support for individuals who need help with daily activities due to aging, illness, or disability. Services may include assistance with bathing, dressing, meal preparation, medication reminders, and light housekeeping, helping individuals maintain independence, comfort, and safety.
